Understanding Graves Disease
Graves Disease is an autoimmune disorder that causes the thyroid gland to produce too much thyroid hormone, a condition known as hyperthyroidism. Named after Irish physician Robert James Graves, who first described the condition in 1835, it remains one of the most common causes of hyperthyroidism worldwide.
What Causes Graves Disease?
In Graves Disease, the immune system creates antibodies called thyroid-stimulating immunoglobulins (TSI) that attach to thyroid cells. These antibodies mimic thyroid-stimulating hormone (TSH), causing the thyroid gland to produce excess thyroid hormones T3 and T4. The exact trigger for this autoimmune response remains unclear, but researchers believe a combination of genetic predisposition and environmental factors plays a role.
Risk factors include family history of autoimmune disease, being female (women are 7-8 times more likely to develop it), smoking, pregnancy, and emotional or physical stress.
Common Symptoms
Graves Disease can affect multiple body systems. Common symptoms include:
- Anxiety and irritability — often the first symptoms noticed
- Unexplained weight loss despite normal or increased appetite
- Heat intolerance and excessive sweating
- Rapid or irregular heartbeat (palpitations)
- Trembling hands and muscle weakness
- Fatigue and difficulty sleeping
- Changes in menstrual cycles
- Graves ophthalmopathy — bulging eyes, eye pain, vision changes
Diagnosis
Diagnosis typically involves blood tests measuring TSH (usually low or suppressed), free T3 and T4 (elevated), and thyroid antibodies (TSI or TRAb). Radioactive iodine uptake tests and thyroid scans may also be used to confirm the diagnosis and assess the gland's activity.
Treatment Options
Treatment approaches include:
- Anti-thyroid medications (methimazole, propylthiouracil) to reduce hormone production
- Radioactive iodine therapy to destroy overactive thyroid cells
- Thyroid surgery (thyroidectomy) to remove part or all of the gland
- Beta-blockers to manage symptoms while other treatments take effect
